Information Consent Form

Dear Families,
We hope this letter finds you well. As part of our most recent quality audit, we received a minor non-conformity with an identified improvement relating to how we obtain and maintain participant consent. Specifically, the recommendation was to:
“Develop a process to ensure each participant’s consent is obtained to collect, use and retain their information or to disclose their information (including assessments) to other parties, including how the service will ensure consent will remain current.”
While much of this is already covered within our Privacy Policy and the YR SPACE Program Privacy Notice, it is necessary for us to implement an additional step to make this process clearer, more consistent, and to ensure consent remains current.
To address this, we have developed an Information Consent Form. This form:
Outlines how information may be collected, stored, and used;
Provides families with the opportunity to nominate which external providers (such as support coordinators, therapists, or your child’s school) YR SPACE may share information with; and
Ensures participants and families are aware of their rights to access, correct, withdraw, or amend their consent at any time.
This form was POSTED to you or SENT HOME IN YOUR CHILD'S BAG (if your child attends Yarra Ranges SDS) on August 28, 2025.

We kindly ask you to review the Information Consent Form, along with the Privacy Policy and the YR SPACE Program Privacy Statement, and return the completed form at your earliest convenience.
