Dear Families and Service Users;
Did you know that there was a 'Feedback' section on our website?
The opinions of service users and their families are important to indicate how well our processes and programs are meeting the needs of the participants, and allows us to continually strive to improve as a service. Your responses to these surveys are entirely anonymous and your privacy is respected.
We invite you to complete the following surveys:
YR SPACE Annual Survey (based upon the previous 12 months)
YR SPACE Program Survey (can be completed at any time, generally relates to the programs/activities offered)
Further feedback can be provided at any time via email, telephone or in person.
